Welcome To Our Office!

Thank you for choosing our office!  Please arrive 30 minutes early for your first visit.  This will give you time to fill out medical information about yourself.  You can save some time by printing out our office forms from this website, fill them out, and bring them with you.  If printing the forms is not convenient, we can mail them to you a few days in advance of your appointment.  When you sign in at the office, our friendly office staff will greet you and enter any insurance information you have.  Please bring all your insurance cards for the first visit.

We are conveniently located at 1749 S. Raccoon Rd. in Austintown , at the corner of Kerrybrook and Raccoon. The office is located in a medical building that is next to the Wedgewood Plaza.  If you need driving directions, please don't hesitate to call us at 330-799-3383.

If you would like to make an appointment, please contact our office by telephone at 330-799-3383 .  Same day emergency appointments are available.

Financial

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits.  We are participating providers with most insurance. We will do everything we can to help you afford the treatment you need and want.  Forms of payment accepted by the office are check, cash, or any major credit card.

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
